Creation and Provision of Anonymous Processing Information

Based on appropriate processing methods based on laws and regulations, the Company continuously creates anonymously processed information that prevents individuals from being identified and prevents the personal information from being restored. The Company provides the anonymously processed information for use in health services, epidemiological studies, and actuarial services, etc. The items of information included in the anonymously processed information created and provided are as follows.。

Items of information included in data provided by insurers

  • Policyholder information (gender, date of birth (delete if 100 years old or over), credentials related to health insurance (policyholder themselves/family status, timing of taking out policy, timing of canceling policy))
  • Receipt data (medical reimbursement statements obtained from the institution that paid for examination)
  • Medical checkup/examination data (results of medical checkups/examinations of policyholder retained by the insurer)
  • Nursing care data (physician’s written opinion on the requirement of nursing care, nursing care (if support is required) certification survey form)

Items of information included in data provided by medical institutions

  • Patient information (gender, date of birth (delete if 100 years old or over), family history, allergies, lifestyle, occupation classification)
  • Receipt data (medical reimbursement statements submitted to institution that paid for examination (medical hospitalization, medical outpatient, DPC, dentistry, home nursing, nursing care))
  • Medication data (medication data including out-of-hospital prescriptions prepared by medical accounting system or electronic medical record)
  • DPC survey data (Form 1, Form 3, Form 4, D file, inpatient EF, outpatient EF, H file to be submitted to Ministry of Health, Labour and Welfare)
  • Test result data (health checkups, medical checkups, in-hospital tests, outsourced blood tests, pathological tests, ophthalmic tests, dental tests)
  • Nursing care data (physician’s written opinion on the requirement of nursing care, nursing care (if support is required) certification survey form)
  • Electronic medical records other than those stated above and data created by in-hospital systems to accompany the electronic medical records (drugs, medical history, prior history, medical examination/treatment, diagnosis, biological information, nursing, rehabilitation, nutrition, falls, blood pressure, cancer registries, death certificate, diagnostic imaging)

Items of information included in data provided by PHR

  • User information (gender, date of birth (delete if over 100 years old), credentials related to health insurance (policyholder themselves/family status, timing of taking out policy, timing of canceling policy))
  • Vital data (steps, body fat, BMI, blood pressure, resting heart rate, sleep time, blood sugar level)
  • Diet questionnaire record (number of snacks, eating speed, eating hours, alcohol consumption)
  • Exercise questionnaire record (frequency of exercise per week, amount of exercise per day, type of exercise performed)
  • Questionnaire record of unexplained physical symptoms (heart palpitations and shortness of breath, stiff shoulders, dizziness, insomnia, stress)

※Items other than the above (address information, postal code, name, telephone number, various ID numbers, etc.) are not included.

The method of providing anonymously processed information is as follows.

  • Send electronic files protected by password via electronic communication means or physical media such as DVD
